Premiums

Benefits and Insurance

Dental crown: As of August 10, 2012, $1,250 annual amount; $1,750 as of January 1, 2013, and $2,000 as of January 1, 2014 [new provision].

Vision care: As of January 1, 2013, coverage will increase to $375.00 [$350.00] and to $400.00 as of January 1, 2014.

Physiotherapy, massage therapy and chiropractor: As of January 1, 2013, $65.00 [$50.00] per visit, to a maximum of $600.00 [$500.00] and a maximum of $700.00 as of January 1, 2014.

Speech and language pathologist and psychology clinic: As of January 1, 2014, $65.00 [$50.00] per visit and maximum of $600.00 [$500.00] per year.

Benefits at age 65: As of January 1, 2012, employees who meet the eligibility requirements are entitled to a health care expense account of $2,500 per year immediately following their 65th birthday, for a period of 10 [5] years.

Meal Allowances

As of July 1, 2012, $10.00 [$7.00] breakfast; $15.00 [$10.00] lunch; $25.00 [$15.00] dinner.

Training Allowance: $55.00 per training day if meals are not provided [new provision].
